SUBJECT: Clarification of Follow-up Form and Process
PURPOSE:
The purpose of this Information Bulletin is to provide clarification on the Follow-up process and the use of the MIS Follow-up form.
BACKGROUND:
Follow-up is one of the necessary and important tasks of the WIA system design. Follow-up maximizes success for your WIA customers, success for your site in meeting Local Area Performance Measures, and success for the Local Area as well.
Follow-up seems straight forward, but there are factors that often cause confusion, which create misunderstanding when completing the follow-up forms submitted to MIS.
Therefore, the MIS staff has spent considerable time in developing an instruction sheet to accompany the MIS Follow-up Contact Information Form and a follow-up tool to assist you in managing the follow-up by quarters flow.
The attachments are:
1. The WIA Follow-up Contact Information Form (EWIF) updated 11/05 to include the Supplemental Data for 2nd Quarter Box used in Adult/DW Retention and Adult/DW Earnings Increase for Local Area Performance.
2. The full Instructions for Completion of the WIA Follow-up Form 3. The Follow-up by Quarters Form to use in each file, if you'd like, to easily see when the follow-up date is for 1st, 2nd, and 3rd quarters based on the quarter of exit, and the time frames for reporting attainments for each follow-up quarter.
4. A graphic matrix of the Follow-up by Quarters information.
The important highlights:
· The follow-up date is always the FIRST day of a quarter. It is ONLY one of 4 dates: January 1st, April 1st, July 1st, or October 1st. It cannot be any other date.
· You have the entire quarter to complete the follow-up interview. The follow-up interview should occur between the 1st day of the quarter and the last day of the quarter. You have till the end of the quarter to submit the MIS follow-up form.
· When you conduct your follow-up interview you obtain information ONLY about attainments the customer had for the quarter you are doing follow-up on. Many of you are giving MIS information that goes back to before the exit, or what happened in future quarters. Each follow-up quarter stands alone as a snapshot.
It is the intention of the MIS department to provide you with as much information as possible so you can accurately complete the MIS follow-up form. Misunderstandings of the proper way to complete the form make delays for yourself and MIS staff.
